Aetna's Formulary for Testosterone Medications
Aetna's prescription drug formulary places generic testosterone cypionate — the injectable form most commonly used for TRT — at Tier 1 (Preferred Generic). This is the lowest copay tier on Aetna's formulary, meaning most Aetna members pay very little for their monthly testosterone medication.
- Tier 1 — Preferred Generic: $0–20 copay per fill. Generic testosterone cypionate injectable. Most Aetna plans place it here.
- Tier 2 — Non-Preferred Generic: $25–40 copay. Some plan variations may tier certain testosterone formulations here.
- Tier 3 — Preferred Brand: $50–100 copay. Brand testosterone products like Testim or Androderm may land here.
What is NOT covered: compounded testosterone. Aetna's policy explicitly classifies compounded testosterone as experimental and investigational — the same designation used for unproven treatments. This means telehealth TRT companies that ship compounded testosterone cannot bill Aetna, and members pay the full out-of-pocket cost. FDA-approved injectable testosterone filled at a standard pharmacy, by contrast, is covered at Tier 1.
Aetna Prior Authorization: Clinical Policy Bulletin 1014
Aetna requires prior authorization for testosterone therapy, governed by Clinical Policy Bulletin (CPB) 1014 and supplemented by Testosterone/Gonadotropin Combination (TGC) PA Policy 976-A. These documents define the clinical criteria that must be documented before Aetna will approve coverage.
Aetna's PA criteria require documentation of all of the following:
- Low total testosterone on two separate morning draws: Both must be collected before 10 AM and show levels below the clinical threshold (generally 300 ng/dL).
- Free testosterone testing (if applicable): For patients with BMI above 30 or elevated SHBG, Aetna may require free testosterone measurement in addition to total.
- Clinical symptoms of hypogonadism: Documented symptoms — fatigue, decreased libido, ED, muscle loss, depressed mood — must appear in the medical record.
- PSA and DRE baseline: Men with risk factors for prostate disease require a recent PSA and digital rectal exam before TRT initiation.
- Ruling out reversible causes: Thyroid function, prolactin, sleep apnea screening, and medication review must be documented to exclude secondary causes.
About "age-related hypogonadism": Aetna's CPB 1014 uses the term "age-related hypogonadism" (also called late-onset hypogonadism) in its policy language — and some patients worry this means coverage is denied for older men. It does NOT. The term describes the medical condition; it does not exclude older patients from coverage. When the clinical criteria are met and properly documented, Aetna covers TRT regardless of age.
What Aetna Covers for TRT Patients
FDA-Approved Injectable Testosterone
Generic testosterone cypionate (the standard injectable form) is covered at Tier 1 on most Aetna plans — typically $0–20 per monthly fill after your deductible is met. This is the form we prescribe most frequently at Revive and the one most reliably covered at the lowest cost tier.
Diagnostic and Monitoring Lab Work
Blood tests for diagnosing and monitoring hypogonadism are covered under your medical benefit (not your pharmacy benefit). Total testosterone, free testosterone, LH, FSH, prolactin, estradiol, PSA, CBC, and metabolic panels are all medically necessary and covered when ordered with proper documentation. Aetna uses Quest Diagnostics and LabCorp as preferred lab partners.
Fertility Preservation Counseling
Notably, Aetna's clinical policy for TRT specifically mentions fertility preservation counseling as part of the pre-treatment discussion. Men of reproductive age should be informed that exogenous testosterone suppresses sperm production. Aetna supports documentation of this counseling. Aetna PPO Plans
PPO plans offer the broadest flexibility. You can see any licensed provider without a referral and fill prescriptions at any in-network pharmacy. Why Telehealth TRT Doesn't Work With Aetna
This is the single most important point for Aetna members to understand. Telehealth TRT companies — Hone, Peter MD, Marek Health, Fountain TRT, and similar platforms — use specialty compounding pharmacies to prepare and ship testosterone directly to patients. Aetna's clinical policy explicitly classifies compounded testosterone as experimental and investigational. Aetna will not cover it.
That means every dollar you pay a telehealth TRT company comes entirely out of your own pocket — despite having Aetna coverage that would pay for the same active ingredient (testosterone cypionate) if it were dispensed through a standard pharmacy as an FDA-approved medication.
