Terms of Service
Please read these terms carefully. They govern your use of Revive Low T Clinic's services, website, and patient portal.
Effective Date: March 20261. Services Description
Revive Low T Clinic ("Revive," "we," "us," or "our") provides physician-supervised men's health services in Washington State, including:
- Testosterone Replacement Therapy (TRT): Comprehensive hormone evaluation, laboratory testing, treatment planning, and ongoing management of hypogonadism and related conditions
- Erectile Dysfunction (ED) Treatment: Diagnosis and treatment of erectile dysfunction, including prescription of FDA-approved medications
- Medical Weight Loss: Physician-supervised GLP-1 receptor agonist therapy (including semaglutide) and related metabolic management
- Peptide Therapy: Physician-prescribed peptide protocols for recovery, hormone optimization, and related conditions
- Telehealth Follow-Up Visits: Virtual appointments for established patients for protocol adjustments and ongoing monitoring
All clinical services are provided by or under the direct supervision of licensed Naturopathic Doctors (NDs) licensed by the Washington State Department of Health. Our physicians hold prescriptive authority under Washington State law.
By scheduling an appointment, using our website, or receiving services, you agree to these Terms of Service.
2. Telehealth Scope — Washington State Only
Revive Low T Clinic provides services exclusively to patients located in Washington State. Initial consultations require an in-person visit at one of our clinic locations. Telehealth follow-up visits are available to established Revive patients who reside in Washington State.
By scheduling a telehealth appointment, you represent and warrant that:
- You are physically located in Washington State at the time of the appointment
- You are an established Revive patient (i.e., you have completed at least one in-person visit)
- You understand that telehealth visits are follow-up appointments and do not substitute for required in-person examinations
Telehealth services are provided in compliance with Washington State telehealth laws (RCW 48.43.735 and related statutes) and applicable Medicaid, Medicare, and commercial insurance telehealth coverage rules.
3. Not an Emergency Service
Revive Low T Clinic is not an emergency medical service and cannot provide emergency care. If you are experiencing a medical emergency, call 911 immediately or go to your nearest emergency room.
Examples of medical emergencies that require 911 or emergency room care include:
- Chest pain, shortness of breath, or signs of heart attack or stroke
- Severe allergic reaction (anaphylaxis)
- Serious injury or trauma
- Thoughts of self-harm or suicide — call 988 (Suicide and Crisis Lifeline) or 911
While Revive physicians are available during business hours for urgent questions from existing patients, we do not provide 24/7 on-call coverage. For after-hours urgent medical concerns, contact an urgent care facility, your primary care physician's after-hours line, or call 911.
4. Patient Responsibilities
As a Revive patient, you agree to:
Accurate Information
- Provide complete, accurate, and current information about your health history, medications, supplements, and symptoms
- Notify your Revive physician promptly of any changes to your health status, medications, or contact information
- Not withhold information that may be relevant to your care or safety
Appointment Attendance
- Attend scheduled appointments as agreed or provide at least 24 hours' advance notice to cancel or reschedule
- Arrive for blood draw appointments in a fasted state (8–12 hours, water permitted) unless otherwise instructed
- Bring required materials to appointments (insurance card, ID, prior lab results if requested)
Medication Compliance
- Use prescribed medications only as directed by your Revive physician
- Not share prescribed medications with any other person
- Store medications according to labeled instructions
- Notify your physician immediately of any adverse effects or concerns
Follow-Up Lab Work
- Complete required follow-up laboratory testing on the schedule recommended by your physician
- Understand that continuation of TRT or other hormonal therapies requires regular lab monitoring and physician oversight
5. Financial Responsibility
Payment at Time of Service
Payment for clinical services (membership fees, visit fees, lab fees) is due at the time of service unless a prior billing arrangement has been made in writing. Revive accepts major credit cards, debit cards, FSA, and HSA cards.
Membership Fees
Membership plans (Standard, Essential, Complete) are billed monthly as agreed at enrollment. Membership fees are non-refundable for the current billing period. You may cancel your membership with 30 days' written notice; cancellation does not affect your outstanding balance for services already received.
Agreement to Pay
By receiving services at Revive Low T Clinic, you agree to pay all charges associated with your care. In the event of non-payment, outstanding balances may be referred to a collections agency and may affect your credit. You agree to reimburse Revive for reasonable costs of collection, including attorney's fees, where permitted by law.
No-Show Policy
Appointments not cancelled with at least 24 hours' advance notice may be subject to a no-show fee of up to $50. Repeated no-shows may result in discharge from the practice at Revive's discretion.
Good Faith Estimates
Under the federal No Surprises Act, you have the right to a written Good Faith Estimate of expected charges before receiving care. See our Good Faith Estimate page for details. If your actual charges exceed your estimate by $400 or more, you have the right to dispute the bill within 120 days.
6. Prescription Coverage Assistance
Pharmacy Prescription Coverage
Revive Low T Clinic assists patients across all membership tiers in submitting pharmacy benefit claims for covered testosterone medications and other prescriptions to their health insurance plan. We make best-effort attempts to submit accurate and complete claims. Insurance coverage determinations are made by your insurance plan, not by Revive.
Patient Financial Responsibility
You are responsible for all amounts not covered by your insurance, including deductibles, copayments, coinsurance, and non-covered services. We will provide you with an explanation of any charges not covered by insurance. You are ultimately responsible for payment of all services received, regardless of insurance coverage status.
Prior Authorization
Some insurance plans require prior authorization before covering certain medications. Revive will assist in obtaining prior authorizations when clinically indicated, but we cannot guarantee approval. If a prior authorization is denied, you may still receive services at the applicable self-pay rate.
Clinical Service Fees
Revive's clinical services (physician visits, in-office labs, injection training) are billed directly to patients as a cash-pay practice. We do not bill major medical insurance for office visits. Clinical service fees are published transparently on our Pricing page.
7. AI-Assisted Care Disclosure
Revive Low T Clinic uses artificial intelligence and automated tools to assist our licensed physicians in providing care. You acknowledge and agree that:
- AI tools may assist with documentation, lab result interpretation, scheduling, and administrative tasks
- All AI-assisted clinical outputs are reviewed and approved by a licensed physician before any clinical action is taken
- No clinical decision regarding your diagnosis, treatment, or medication is made by AI alone — all such decisions are made by your licensed Revive physician
- Your de-identified health data may be used in aggregate to improve our clinical operations, subject to HIPAA and applicable privacy law

8. Intellectual Property
All content on the Revive Low T Clinic website, including text, images, graphics, video, and educational materials, is the property of Revive Low T Clinic or its licensors and is protected by applicable copyright, trademark, and intellectual property laws.
You may use our website content for personal, non-commercial purposes only. You may not:
- Reproduce, distribute, or publish site content without our prior written permission
- Use our trademarks, logos, or brand elements without written consent
- Frame or embed our website in any other website without authorization
Patient health records and other PHI remain the property of the patient as provided under Washington State law (RCW 70.02.080). You have the right to access and obtain copies of your records as described in our Privacy Policy.
9. Limitation of Liability
To the maximum extent permitted by Washington State law, Revive Low T Clinic's liability for any claim arising from services provided shall be limited to the amount paid by the patient for the specific service giving rise to the claim.
Nothing in these Terms limits Revive's liability for:
- Professional malpractice or negligence claims arising from clinical care
- Death or personal injury caused by our negligence
- Fraud or willful misconduct
- Any liability that cannot be excluded or limited under Washington State law
Revive Low T Clinic makes no guarantee of specific clinical outcomes. Individual results from TRT, ED treatment, weight loss, and other services vary based on patient physiology, compliance, and other factors outside our control.
10. Governing Law & Dispute Resolution
These Terms of Service are governed by and construed in accordance with the laws of Washington State, without regard to its conflict of law provisions.
Any dispute arising from or relating to these Terms or your use of Revive's services shall be subject to the exclusive jurisdiction of the state and federal courts located in King County, Washington. You consent to personal jurisdiction in such courts.
Before initiating formal legal proceedings, both parties agree to attempt to resolve disputes in good faith through direct communication. Please contact us at (206) 960-4770 or privacy@revivelowt.com to attempt informal resolution.
11. Changes to These Terms
Revive Low T Clinic reserves the right to modify these Terms of Service at any time. We will provide notice of material changes by posting the updated Terms on our website with a revised effective date. Your continued use of our services after such notice constitutes acceptance of the updated Terms.
If you do not agree with updated Terms, you must discontinue use of our services and contact us to discuss your options. We recommend reviewing these Terms periodically.
12. SMS Messaging
Program Description
By providing your mobile phone number and consenting to SMS, you agree to receive text messages from Revive Low T Clinic (message originator: Revive Low T LLC). Messages may include:
- Appointment reminders and scheduling confirmations
- Care follow-up notifications and lab result alerts
- Two-factor authentication (2FA) codes for the patient portal
- Account and billing notifications
- Patient care team communications
Opt-In and Opt-Out
SMS consent is collected verbally during your scheduling call or via the patient portal. Consent is entirely voluntary — you may decline without affecting your ability to receive care.
To stop receiving messages at any time, reply STOP to any text message from us. You will receive a single confirmation and no further messages will be sent.

Message Frequency and Rates
Message frequency varies based on appointment schedule and care activity. Appointment reminders are typically sent 24–48 hours before scheduled visits. 2FA codes are sent on demand during portal login.
Message and data rates may apply. Contact your wireless carrier for details on your plan's messaging rates. Revive Low T Clinic does not charge for SMS messages.
Carrier Disclaimer
Not all mobile carriers are supported. Supported carriers include AT&T, T-Mobile, Verizon, Sprint, Boost Mobile, U.S. Cellular, and other major U.S. carriers. Revive Low T Clinic is not liable for delayed or undelivered messages.
Privacy
Your mobile number and messaging consent are stored securely and are never sold or shared with third parties for marketing purposes. See our Privacy Policy for full details.
