These Terms of Service ("Terms") govern your use of the Revive Low T Clinic website (revivelowt.com) and the medical services provided by Revive Low T Clinic ("Revive," "we," "us," or "our"). By accessing our website or receiving our services, you agree to these Terms.

Scope of Services

Revive Low T Clinic provides outpatient men's health services including:

• Testosterone replacement therapy (TRT) evaluation, diagnosis, and treatment
• Erectile dysfunction (ED) evaluation and treatment
• Medical weight loss with GLP-1 medications (semaglutide)
• Comprehensive lab testing (51-analyte panels)
• Ongoing monitoring and protocol optimization

Our services are provided at three clinic locations: Seattle, Kirkland, and Federal Way, Washington. All care is delivered by licensed naturopathic physicians (NDs) authorized to practice medicine in the state of Washington.

Not Emergency Care

Revive Low T Clinic does not provide emergency medical services. If you are experiencing a medical emergency, chest pain, difficulty breathing, or any life-threatening condition, call 911 or go to your nearest emergency room immediately. Our clinic is designed for scheduled outpatient visits and is not equipped to handle acute emergencies.

Patient Eligibility

Our services are available to adult males aged 18 and older. Treatment decisions are made by our physicians based on clinical evaluation, lab results, and medical history. We reserve the right to decline treatment if our physicians determine that a particular therapy is not appropriate or safe for you.

Appointments & Cancellations

We ask that you provide at least 24 hours' notice if you need to cancel or reschedule an appointment. Late cancellations or no-shows may result in a cancellation fee. Specific cancellation policies will be communicated at the time of booking.

We make every effort to see patients at their scheduled time, but appointment durations may vary based on clinical needs. We appreciate your patience and understanding.

Payment Terms

Payment for membership plans and services is due at the time of service or as specified by your plan agreement. We accept:

• Major credit and debit cards (Visa, Mastercard, American Express, Discover)
• Flexible Spending Accounts (FSA)
• Health Savings Accounts (HSA)
• Online payments through our patient portal

Membership plans (Essential, Standard, Complete) are billed monthly. You may cancel your membership at any time with 30 days' written notice. Refunds for partial months are not provided. Medication costs are separate from membership fees and are billed through your pharmacy.

Insurance & Pharmacy

Revive prescribes FDA-approved medications to the pharmacy of your choice. While many insurance plans cover testosterone and other prescribed medications, coverage varies by plan and carrier. We assist with insurance billing for our Standard and Complete plans, but we do not guarantee insurance coverage. You are ultimately responsible for any costs not covered by your insurance.
