What's Actually Causing Your ED?

Erectile dysfunction is rarely "just in your head," and it's rarely just one thing. It's a signal — and the most common hormonal cause in men over 30 is low testosterone. Testosterone drives the nitric oxide pathway that erections depend on, fuels libido, and supports the penile tissue and confidence that make everything work.

That's the part most clinics never check. They hand you a pill without ever asking why. But ED can come from your hormones, your circulation, your metabolism, or even another medication you're taking — and the right treatment depends entirely on which one it is.

Low testosterone

The most common hormonal cause of ED in men over 30.

Elevated estrogen

Too much estradiol from testosterone aromatization.

Circulation & vascular health

Blood flow plays a key role in erectile function.

Metabolic issues

Diabetes, insulin resistance, and obesity.

Medication side effects

Blood pressure meds, antidepressants, and others.

Performance anxiety

Low testosterone contributes to the mood changes that worsen ED.

Why a Pill From an App Often Isn't Enough

Here's what usually happens before a man finds us.

You fill out a form online. A telehealth company you'll never meet in person ships you a branded compound at a premium — $150–300 a month — and no insurance will touch it. There's no exam, no labs, and nobody asking what's actually wrong.

And the pill doesn't fully work. PDE5 inhibitors are less effective when the underlying cause is hormonal rather than vascular. Many men with treatment-resistant ED have undiagnosed low testosterone or thyroid dysfunction — so they keep paying for a medication that was never going to fix the real problem.

There's a safety cost too: ED medications can interact dangerously with nitrates and certain blood pressure medications. An online form can miss critical interactions a doctor would catch in the room.

You deserve a doctor who finds out why — and is in the room when it matters. That's why Revive exists.

How We Do It

We Treat the Cause First — Not Just the Symptom

Every Revive patient gets a real evaluation before any medication is prescribed.

🧪

A 51-Biomarker Panel

Drawn in our clinic, results in about a day. We test testosterone (total and free), estradiol, SHBG and metabolic markers — diagnosing the hormonal and vascular contributors to ED, not just handing you a pill. The ED & low-T connection →

🩺

An In-Person Exam

You sit across from a licensed doctor, who checks your vitals, reviews your full medication list, and builds a plan around your labs and your goals — face to face. An online form can't do that safely.

💊

Your Local Pharmacy

We send FDA-approved generic medication to the pharmacy you already use — Costco, CVS, Walgreens, or your neighborhood pharmacy — so your insurance formulary applies, instead of mail-order compound.

Then we tailor the treatment to you

Testosterone first ROOT CAUSE

If your labs show low testosterone — the most common hormonal cause of ED — optimizing it is our first-line, evidence-based intervention. Many men taking Viagra or Cialis get better results after optimizing testosterone, or find they no longer need the medication at all.

FDA-approved ED medications

When you'd benefit from added support, we prescribe generic sildenafil (Viagra) and tadalafil (Cialis) — FDA-approved, filled at your local pharmacy, and far more affordable than telehealth compounding models.

Novel & emerging therapies

For PDE5 inhibitor non-responders, we offer evaluation for emerging options including centrally-acting therapies such as PT-141 (bremelanotide) — off-label, with full informed consent disclosed at consultation. Full medication guide →

Done right, it's more than one number

Sometimes the culprit is elevated estradiol from testosterone aromatization — not low testosterone alone. When your labs call for it, we manage estrogen as part of the plan so we're treating the cause, not masking the symptom. See all novel ED therapies →

Revive vs. Telehealth ED

For erectile dysfunction, the difference between a real evaluation and a questionnaire is the difference between treatment that works and a pill that doesn't.

Dr. Barry Wheeler, ND — Revive Low T Clinic, Kirkland
Who You'll See

Two Licensed Doctors Who've Done This Since 2013

I'm Dr. Barry Wheeler. My colleague Dr. Chris Merlino and I are licensed doctors here in Washington, and between us we've sat across from thousands of men having this exact conversation — there's nothing to be embarrassed about. We're a men's health clinic, not a family-practice waiting room, and not a call center. When you book, you see one of us.

The telehealth ads tell you to "consult a doctor." We are the doctor — and your visit, diagnosis, and prescriptions are fully protected by HIPAA, always.

What Changes

What You'll Actually Notice

It depends on the cause. PDE5 inhibitors work fast; treating a hormonal root cause is gradual, then it's obvious. Here's the honest timeline we give every patient — your mileage varies, but this is the shape of it.

Same day

Sildenafil and tadalafil typically work within 30–60 minutes for most men.

Weeks 4–8

When the root cause is hormonal, improvements from TRT come on as your levels normalize.

By month 2–3

Confidence and libido return — many men find they need the pill less, or not at all.

Ongoing

We dial the plan in with repeat labs and keep your treatment effective and safe.

Common Questions

ED Treatment: What Men Ask Most

Can ED be treated without surgery?

Yes — the vast majority of erectile dysfunction cases are treated effectively with oral medication, testosterone therapy, or a combination of both. Surgery is rarely necessary. FDA-approved medications like sildenafil (Viagra) and tadalafil (Cialis) work for most men when the underlying cause is identified.

At Revive, we start by diagnosing the root cause. For many men, low testosterone is a contributing factor that amplifies ED — and treating it with TRT alongside an ED medication produces significantly better results than medication alone. Learn about the ED and low testosterone connection.

What medications treat ED, and what's the difference between Viagra and Cialis?

The most common ED medications are sildenafil (generic Viagra) and tadalafil (generic Cialis). Both are PDE5 inhibitors. Sildenafil works in 30–60 minutes and lasts 4–6 hours, taken on demand. Tadalafil lasts up to 36 hours and can be taken as a small daily dose (2.5–5mg) for spontaneity.

We prescribe FDA-approved generics to the pharmacy you choose — Costco, CVS, Walgreens, or your neighborhood pharmacy. For PDE5 non-responders we also evaluate emerging options such as PT-141 (bremelanotide), off-label with full informed consent. See our full medication comparison guide.

Does insurance cover ED treatment?

Many insurance plans cover generic ED medications like sildenafil and tadalafil, especially when prescribed for a documented medical condition. Coverage varies by plan — generic sildenafil and tadalafil run $20–40/month where covered. Generic options stay affordable even without insurance.

We prescribe to your local pharmacy where your insurance formulary applies. Many commercial plans exclude ED medications for the sexual-dysfunction indication, but tadalafil for BPH (enlarged prostate) is covered by most commercial plans and Medicare Part D. If ED is related to low testosterone, treating both together may increase the likelihood of coverage for the testosterone component — TRT is $0–30/month on most plans. Check your coverage.

How much does ED treatment cost?

It starts with a free phone consult, and your first visit — which includes the in-person exam and the 51-biomarker panel — is $99. From there, the medication itself is inexpensive at a retail pharmacy: generic sildenafil is $10–20/month and generic tadalafil is $10–25/month with a discount card. Our Costco pharmacy cash price for generic tadalafil is under $15/month.

If low testosterone is part of the picture, TRT is $0–30/month with insurance or $14–30/month cash. Ongoing care plans start at $35/month. Telehealth ED companies (Hims, Roman, etc.) charge $150–300/month because they use compounding pharmacies no insurance will cover — Revive prescribes FDA-approved generics to retail pharmacies, making your treatment insurance-eligible and dramatically more affordable. No hidden fees, no surprise bills. See all plans.

What if medications haven't worked for me before?

PDE5 inhibitors are less effective when the underlying cause is hormonal rather than vascular. Many men with treatment-resistant ED have undiagnosed low testosterone or thyroid dysfunction.

Revive investigates the root cause with lab work — if hormones are the issue, treating low T often restores response to ED medications or eliminates the need for them entirely. New patients require an in-person evaluation at one of our three Washington locations (Seattle, Kirkland, Federal Way); established patients can use telehealth statewide for refills and follow-ups.

Done Safely

Why Treating the Whole Man Lowers Your Risk

ED medications can interact dangerously with nitrates and certain blood pressure medications, and ED itself can be an early sign of broader cardiovascular or metabolic issues. The risk was never the treatment — it's treatment handed out without anyone looking at the rest of you.

That's why we run 51+ biomarkers, check your vitals, and review your full medication list in person: so we can prescribe safely, treat the actual cause, and make sure your plan fits the whole picture — not just one symptom.

My promise: we'll tell you honestly where you stand. If you don't need a daily medication, we'll say so. No pressure, no upsell — just a real diagnosis and a plan that actually works, in a clinic built for your privacy and comfort.

Take the First Step — Confidentially.

It starts with a free phone consult — book it online in under a minute. Your first visit includes a sit-down with me or Dr. Merlino, a physical exam, and a free 51-biomarker panel ($400 value). No pressure, no commitment.

P.S. — If you take one thing from this page: a pill from an app treats the symptom and skips the cause. For a lot of men, that cause is low testosterone — and once we find it, the ED often takes care of itself. That's the whole difference between "here's a prescription" and "here's why this was happening."
