Lab Testing

The 51-Biomarker Panel Behind Better TRT.
Drawn In-Clinic. Reviewed by a Doctor.

Most telehealth clinics test 4–7 markers — just enough to justify a prescription. I run a 51-biomarker panel at your first visit, drawn on-site and read by me or Dr. Merlino, so your treatment is built on the whole picture — not a guess.

Why It Matters

Why We Test 51+ Biomarkers
When Others Test 4

Most telehealth TRT companies test total testosterone, maybe a basic CBC, and call it a day. They're testing just enough to justify a prescription — not enough to safely manage your care.

Testosterone therapy affects your entire body — blood viscosity (hematocrit), estrogen balance, liver and kidney function, metabolic health, thyroid, iron, and lipids. Without baseline data, your doctor can't identify pre-existing conditions, predict side effects, or optimize your protocol.

Our 51-biomarker panel gives your doctor the complete picture — the difference between treating a number and treating the whole patient.

What We Test

Everything We Measure — and Why

A full hormonal, metabolic, and safety workup across every system TRT touches.

Hormones

The full hormonal picture — not just a single testosterone number. Reveals why your levels are low and how to treat it safely.

Total Testosterone · Calculated Free Testosterone · Estradiol (E2) · SHBG · LH · FSH · Prolactin · DHEA-S

Complete Blood Count (CBC)

Critical TRT safety monitoring — testosterone raises red blood cell production, so we watch your hematocrit at every visit.

WBC · RBC · Hemoglobin · Hematocrit · Platelets · MCV · MCH · MCHC · RDW · MPV · Neutrophils · Lymphocytes · Monocytes · Eosinophils · Basophils

Comprehensive Metabolic Panel

Kidney function, electrolytes, blood sugar, and liver enzymes — a safe baseline before TRT and ongoing metabolic health.

Glucose · BUN · Creatinine · eGFR · Sodium · Potassium · Chloride · CO₂ · Calcium · Total Protein · Albumin · Bilirubin · AST · ALT · ALP

Heart & Lipids

TRT can shift cholesterol. Monitoring lipids keeps your cardiovascular health on track throughout treatment.

Total Cholesterol · LDL · HDL · Triglycerides

Iron Studies

Low iron causes fatigue that mimics low T, and TRT increases iron demand. We check storage and transport.

Ferritin · Iron (Serum) · TIBC

Thyroid, Blood Sugar & Prostate

Rules out thyroid and diabetes look-alikes for low T, and establishes your prostate safety baseline.

TSH · HbA1c · PSA (Prostate-Specific Antigen)

Calculated Free Testosterone —
The Marker That Actually Matters

Why Total T Alone Misleads

Only 2–3% of testosterone in your blood is "free" — unbound and biologically active. The rest is bound to SHBG and albumin, unavailable for your body to use.

A man with total T of 450 ng/dL and high SHBG can have the free testosterone of someone at 250. Testing total T alone misses this entirely.

Why We Calculate It

We derive free testosterone from Total T + SHBG + Albumin — the calculated method endorsed for accuracy over the cheaper "direct" immunoassay, which is unreliable in men.

It's the difference between a number you can trust and one you can't — and most telehealth clinics skip free T altogether.
