Revive Low T Clinic operates this website at revivelowt.com. We are a men's health clinic with locations in Seattle, Kirkland, and Federal Way, Washington, specializing in testosterone therapy, ED treatment, and medical weight loss.

As a Washington-based business that collects health-related data from Washington residents, we are subject to the My Health My Data Act (MHMDA), RCW 19.373, effective March 31, 2024.

This notice covers data collected through our website only. For information about how we handle your medical records as a patient, please refer to our Privacy Policy and the HIPAA Notice of Privacy Practices provided at your first clinical visit.

2. What Consumer Health Data We Collect

Washington's MHMDA broadly defines "consumer health data" to include information that identifies or is reasonably linkable to a consumer and relates to the consumer's health status, conditions, or use of healthcare services.

We do not collect health data passively from visitors who have not given consent. Analytics tools only activate after you choose to accept them via our consent banner. Quiz responses are only collected when you voluntarily take the quiz and submit your information.

3. How We Use Your Data

We use consumer health data collected on this website for the following purposes:

  • Improving website experience: Analytics data (with consent) helps us understand which content is helpful, identify technical issues, and improve navigation.
  • Marketing analytics: Aggregate, anonymized data about how visitors engage with our pages helps us understand which health topics are most relevant to our community.
  • Responding to appointment requests: When you submit a form or complete the Low T Quiz, we use your contact information and quiz responses to have a care team member follow up with you about booking a consultation.
  • Facilitating clinical care: If you become a patient, information from your initial inquiry may be used to facilitate your care at one of our clinic locations.
  • Regulatory compliance: We may retain certain records as required by Washington state law or for HIPAA compliance purposes.

We do not sell consumer health data. We do not use your health data to make automated decisions that have legal or similarly significant effects without human review.

We share consumer health data only in the circumstances described below:

Appointment booking and clinical care systems

When you submit a contact form or quiz lead, your information is transmitted to our secure appointment booking and patient management system for the sole purpose of facilitating your care. This data is handled under HIPAA-compliant Business Associate Agreements with our technology vendors.

Legal and safety disclosures

We may disclose information when required by law, in response to valid legal process, or to protect the safety of our patients, staff, or the public.

We do not sell, rent, or trade consumer health data to data brokers or unaffiliated third parties for commercial purposes.

5. Your Rights Under MHMDA

Washington's My Health My Data Act grants Washington residents the following rights with respect to consumer health data we hold about you:

Right to Know

You have the right to know what consumer health data we hold about you and how it is being used or shared.

Right to Access

You have the right to request a copy of the consumer health data we have collected about you.

Right to Delete

You have the right to request deletion of your consumer health data from our systems and from our service providers.

Right to Withdraw Consent

You may withdraw consent for analytics or health data sharing at any time. Withdrawal is effective going forward — it does not affect past data processing.

Right to Refuse Sale

We do not sell consumer health data. If that changes, you will have the right to opt out before any such sale occurs.

Right to Non-Discrimination

We will not deny you services, charge you different prices, or provide lower quality care because you exercised any of these privacy rights.

Response timeline

We will respond to verifiable consumer requests within 45 calendar days of receipt. If we require more time, we will notify you of the extension within the initial 45-day window. In limited circumstances where a request cannot be fulfilled, we will explain why and describe any appeal process available to you.

Withdraw or manage your analytics consent

You can change your analytics and health data sharing preferences at any time using our consent manager.

6. How to Submit a Privacy Request

To exercise any of the rights described above — including requests to know, access, delete, or withdraw consent — please contact our Privacy Team by email. Include your full name, the email address associated with your request or account, and a description of the specific right you are exercising.

Verification

To protect your privacy and prevent unauthorized access, we may need to verify your identity before fulfilling a request. We will use the minimum information necessary for verification and will not use verification information for any other purpose.

Authorized agents

You may designate an authorized agent to submit requests on your behalf. We may require documentation verifying the agent's authority before processing the request.

Appealing a decision

If we decline to take action on your request, you may appeal our decision by replying to our response email with the subject line "Appeal — MHMDA Request." We will review appeals within 45 days and provide a written explanation of our decision. If your appeal is denied, you may file a complaint with the Washington State Attorney General's Consumer Protection Division.
